23 Changes to the Online Employee Claim:  You can now check the status of your online employee claim.  A new tab is added to the "Start a Claim"  and "Complete a Claim" options.  "Check Form Status" allows you to check the status of any claim started for 45 days after initiated.  If the claim submission is approved, you can also obtain the WCC Claim number in the results.
16 Updated Form – Stipulation of Parties and Award of Compensation WCC form H-34 (04/2018) has been changed. The revised form is available on our Forms and Instructions page as a fillable PDF. Please begin using the new form immediately.
