Basic Electronic Form Filing Instructions For WFMS Subscriber WebForms Please read and follow all instructions before accessing and using WebForms.
Subscribers must enable "session cookies" in their browser and include the MD WCC "Domain"
https://services.wcc.state.md.us in their "trusted sites" to allow
trusted access in their Internet browser. Different browsers, e.g. Mozilla Firefox may implement their security settings differently.

TO DOWNLOAD AND INSTALL FORMATTA FILLER 7.0
In order to utilize WCC WebForms to securely submit online to the WCC, you must install Formatta(c) Filler (current version) software on your PC. Formatta Filler is designed for Microsoft Windows (c). The most current version of Internet Explorer is recommended for proper functionality. Please read all instructions prior to beginning to install Formatta Filler. |
Select "Open" when prompted to download the
"fillersetup.exe" and follow the on screen installation
instructions. If using a business/corporate PC, you MUST have
access/rights to install software for Filler to function. If
you do not have the right to install software on your PC, please
"Save" instead of "Open" and contact your IT support to install
this software. Formatta Filler is a small, light application
that works with your Internet browser to file forms or alone to
view saved submitted forms. After installation, Formatta Filler
will launch a registration form. Registration is not required
but will provide some additional support from the software
provider. You should become
familiar with Formatta Filler, view the toolbar functions (at the top
of the page) and also utilize the "Auto Update" functions found
in the HELP listing from the toolbar.
All WCC WebForms require Formatta Filler
7.0. Forms that include a
"Submit" button actually submit the required form and data to
the Commission/WCC online via your Internet connection. You
cannot save a "copy" of a blank online WebForm to your PC for
re-use. The form must be selected from the WCC web page each
time a form is submitted. Each form in assigned a unique name
with each request. A saved completed WebForm cannot be
re-submitted. Completed WebForms can be saved for viewing or
printing at any time but saved forms are viewed ONLY in Formatta
Filler. You cannot view the form in Microsoft(c) Office Word,
Adobe® Reader or other office applications. Use the Formatta
Filler toolbar which includes "Print", "Save", "Save As"
buttons/functions to save a completed/submitted form to your PC
for future print or reference. Adobe® Acrobat or other software
that acts as a printer driver can be used to print the completed
form to your PC. | |

Forms that require or
indicate by certification of service that copies be sent
by you, the originator to
other parties are your responsibility to print and mail them; WebForms do not change this basic form requirement. The
WCC will not send copies of the WebForm to the required
parties. You must Print and/or Save your form after submission
since you will no longer have access on your PC to the
completed form after you have closed Formatta Filler (if you
have not printed or saved the form). Your signature in these
cases attests to this certification in the form's text
instructions (on the form).

Every WebForm submission will result in an email receipt
sent to your registered email address. The email will indicate
that your form submission has succeeded or failed. If your form
submission fails, the email instructs that you MUST resubmit the
form. If you do not receive this email, your mail server
policies or SPAM solution may simply be blocking these emails
from the Commission. Failure or refusal of emails to your
registered email address will result in suspension of access to
Online Services.
You may view your filed forms via the WFMS Claim
Documents list immediately after a successful submission to confirm your
WebForm is processed.

All WFMS Online Services
inquiries and forms require that two identifying elements are provided in the
listed input fields. Generally the available choices are: WCC Claim Number,
Social Security number, surname and date of birth (MM/DD/YYYY format).
The
claimant last or surname input should be entered as a "wildcard: search; enter
ONLY the first three (3) letters of the last name. Remember that you are searching through structured
information and your search must match our data to provide a successful result.
The WCC Claim number + surname or Social Security number + surname are generally
provide the best results.

Be sure to select the correct form, enter the correct
information and attached the correct documents as attachments.
We cannot disregard, cancel, recall or
delete forms filed online. Submitted forms are
entered on receipt into the WCC electronic processes.
Please file the appropriate form to accomplish the intended
result or contact the WCC Claims Division
for clarification of administrative procedures and policies.

Forms permitting attached files: Select the form's "Attach"
button. All documents/files attached to a WCC form must be
designated as "Secured" or "Unsecured". A dialog box will
display to define these terms. It is the responsibility of the
subscribed Attorney to provide such description of the attached
documentation as outlined in the dialog box text. Once these
terms are accepted, a new "Attachments" dialog box will display
to allow you to browse to the files to be attached from your PC.
You may attach as many files as necessary but some judgment and
discretion should be used. When scanning a document for
attachment, make sure that the scan quality is readable, that
the image when added to a document (e.g. Word.doc) as an image
is FULL SIZE and not reduced to make the document unreadable. Scanning of documents requires some experience and
expertise to properly accomplish.

You may not attach a Formatta form (*.pff
file) to an online WebForm

If your form submission fails twice you must
discard the form and generate a new form.
Generally, most forms will submit successfully
on the second attempt.

Do not mail or deliver a copy of the completed WebForm to the
Commission.

Common Office application formats, e.g. Microsoft(c) Office Word
(.doc), graphics (.jpg, .gif, .jpg, .bmp, .tif), Adobe® (.pdf),
WordPerfect(c) (.wpd), MS Excel (.xls), are allowed.
Formatta forms (files ending in the .pff extension) cannot be
attached to Formatta forms. Files
attached must use a valid file type and "syntax": this includes
the file name (name) and file type extension, like filename.doc
(MS Office Word). Files without a valid file extension or
syntax, e.g. "EHR attachments" or file.myfile, cannot properly
process and will not be accepted. Incorrect/invalid attachment
(file) types will generate a failure email, correct submissions
will receive a confirmation email. If you have a question

websupport@wcc.state.md.us prior to attempting submission. Once
the attachments are complete, select "Close" and submit the
completed form with the attachments. You may "Delete" files that
are attached in error prior to submission of the WebForm. "Save
As" only saves a copy of the attached file back to your PC. A
successful attached file will indicate a “gold paper clip” icon
in the upper right Filler toolbar, a grey “paper clip” indicates
no file is attached. No file attached should exceed 5MB in size.
Average page file size as Adobe® PDF or word processing
documents is 24KB or less. Scanned images instead of word
processing documents may be larger but no properly scanned
document should exceed 250KB or about 1/4 MB.

Required
information fields on all WebForms are the same as paper forms submitted to
the WCC. WebForms have and meet the same legal
requirements as hardcopy forms submitted.

Like most web-based forms, WebForms
will proceed through the available data input fields. When
possible, the form will pre-populate with the claim data.
When possible, form fields will validate the information you
enter- for example if selecting "yes" or "no" indicates you
must explain or provide more information, you cannot submit
the form without input into the "required" field.
Each field will display an "information bubble" or tip when
your cursor is in the field to explain the information
requirements.

WCC WebForms are selected
from a list of available forms and will require that the MD
WCC Claim Number and one other indicated element (WCC Claim Number, Social Security number or date of birth)
is input and then Filler will then launch the form. If the
wrong claim data is entered and the claim information
displayed is not the one desired, close Filler and verify
the MD WCC Claim Number*.

At SUBMIT, you will enter
your WFMS Password to "sign" the form, your WFMS
username/login should be pre-filled in the digital signature
dialog box. If you enter the password incorrectly or an
incorrect password, the signing dialog box will present
again until the correct password is entered.
By signing the form, the subscribed Attorney attests that
they are the registered WFMS Attorney and agree to the posted Terms of Service agreement. Successful
submission of the form will send a confirmation receipt with
the form ID number, date and time to the WFMS-registered
email address. When ready to "submit" the form to the WCC:
verify the form information, select "Submit". The results
of a successful submission will display the completed form
with a "time stamp" and your name as registered with WFMS
and registered email address. This is the version to
Print/Save.
If you have the wrong WCC claim number, please utilize WFMS to "Search" via the Claim Inquiry to obtain the correct WCC Claim Number. WFMS Websupport does not research claim numbers.

Selection
As part of our WFMS Online Information security initiative, the dual-data input security requirement utilized for claim information search and document view
is also implemented for our online WebForms. To launch a WebForm, each subscriber
is required to provide the claimant last (sur)name and the WCC Claim Number or Social Security number or Date of Birth.
A sample image of the interface is provided at right.
Remember to use the "Wildcard"
search method for all name inputs: first three letters and an asterisk (e.g. smi* for smith) for better and faster results.

Updates/Installations FORMATTA FILLER
SHOULD BE UPDATED via the Filler Update feature in Formatta Filler's "HELP" menu to function after the installation or upgrade of Internet Explorer
7.x or later. If you experience a "server connection error" message, this is the indication that you MUST update your version of Formatta Filler to be compatible with new security features in IE 7.x. Launch Formatta Filler from your Programs list and use the Update Filler option in the Help menu.
Formatta Filler Updates should be performed. as part of a
routine maintenance.
