17 New Form - Claimant Request for Change of Address WCC form H31R (01/2016). This form is to be used ONLY to change the Claimant Address for the claim number indicated on the form. The new form is revised and available for electronic filing through Online Services subscribed Claimant Attorney/Proxy. The fillable PDF form on our Forms and Instructions page is also updated. The Commission does not accept form filing via email/attachment or fax. Please begin using the new form immediately.

The Bar Association of Montgomery County Workers Compensation Section and the Maryland Workers Compensation Commission invite you to join us for this important symposium addressing the opioid crisis being faced by the workers’ compensation community. Panelists include medical professionals from the pain management and rehabilitation community, Commissioners, and attorneys who will speak on the issue and lead us in discussions with an eye toward a solution.  ALL ARE WELCOME.
